| Outline of Final Research Achievements |
Community-based integrated care system is in need of outcome. This study will develop the translation of the ‘ASCOT’ (Adult Social Care Outcome Measurements Toolkit developed by the Univ. of Kent) into Japanese and verifying it, integrate items of Japanese version ASCOT into the local governments’ survey of older people including Long-term care users, and investigate how the social care related quality of life data is able to be utilized for effective management of long-term care, especially community-based integrated care, at the local administration level as well as clinical practice level in Japan. The Japanese version of ASCOT SCT4 that has linguistic and statistical validity was successfully established. With regard to practical use of the measurement, under the collaboration with municipalities and clinical care specialists, benefits and efficacy of the established measurement in evidence-based management of care was confirmed.

| Academic Significance and Societal Importance of the Research Achievements |
ケアの「社会的側面」「利用者にとっての生活の質」「アウトカム」という要素は、これまで重要だが政策評価において軽視されてきた。本研究では、これらの要素を含むケア評価がメインストリーミング化した社会の実現に貢献すべく、そこから遡って、設計科学的アプローチで「ケアを可視化するツール」としての社会的ケア関連QOL尺度を開発した。また、当該ツールにより把握されるケアデータをエビデンスとした、これから求められるオルタナティブなケアシステム運営の実装に貢献した。
