Research Abstract |
We sent, in 1990, questionnaires to the major dermatological institutions in Japan concerning the course and associations of dermatomyositi-s and could obtain responses from 186 institutions. We analysed the total 237 cases (182 cases following the criteria of Bohan & Peter, definite or probable dermatomyositis) diagnosed by dermatologists from Jan. 1983 to Dec-1987. In 17 children, no case died, , In adults, 44 cases (26.7*) died : the causes of death were attributed to malignancies (16 cases), interstitial pneumonitis (13 cases), other pneumonia (4 cases), and so on. The associated malignancies were gastric cancer, lung cancer, breast cancer and so on. -Therewas only-a case of malignant lymphoma. The mortality due to cancer was 34.0%. Periungual erythema and dysphagia were more frequently associated in cancer group (p<0.01). Pruritus and poikiloderma were more frequently noted in cancer group (p<0.05). Interstitial pneumonitis (IP) was associated in 31 cases after the age of 30, and 13 of them died of pneumonitis : the majority died within 6 months after the diagnosis of dermatomyositis. Skin ulcer and arthralgia were more frequently noted in the IP group, while pruritus was less frequently associated (p<0.01). No serum examination was statistically different between the IP and non-IP group, but upper dermal edema in skin biopsy was more frequently noted in the IP group. Thus, we could obtain some predictive factors for the vital prognosis of dermatomyositis. In contrast, the association of cancer in systemic lupus. erythematosus in our department was only 3 cases (6.8%) out of 44 cases much lower than the incidence (30.1%) in adult dermatomyositis.
