Research Abstract |
Multiple myeloma has been investigated with a special view to cell-biological aspects of myeloma cells. A new strategy to the management of multiple myeloma may be obtained from these investigations. 1. It was electron-microscopically demonstrated that the lysosomal system played a major role in vacuolation of myeloma cells. 2. An unusual case of multiple myeloma with double Bence Jones proteinuria was studied. Cells off common clonal origin produced Bence Jones proteins of both kappa and lambda type. 3. The expression of myeloid antigen on the membrane of myeloma cells may be related to the occurrence of plasma cell leukemia. 4. The incidence of ringed neutrophils in multiple myeloma seemed to vary according to disease state. Ring neutrophils may reflect abnormal granulopoiesis in plasma cell dyscrasias, including multiple myeloma. 5. Significant uptake of technetium-99m (V) DMSA was found in the deposition of amyloid. Technetium-99m (V) DMSA scintigraphy could be useful in determining the appropriate region of biopsy and in forecasting the prognosis of myeloma patients. 6. A number of different combination regimens including high-dose corticosteroids (HDCS) have been widely used in an attempt to achieve better results for relapsed or alkylating agent-resistant multiple myeloma. A rapid progression of systemic amyloidosis is supposed to be attributable to the HDCS therapy. 7. The isoelectric point of Bence Jones protein may be related to the amyloidogenecity of Bence Jones protein and neoplastic nature of plasma cells producing Bence Jones protein.
