Research Abstract |
We performed fetal preparation surgery on 9 ewes. The pregnant ewes were operated at gestation ages of 120 - 140 days (term = 145 days). Anesthesia was induced with sodium thiopental, the ewes were intubated with cuffed endotracheal tube and anesthesia was maintained by the GOF method. By hysterotomy the left side chest of the fetus were exteriolized Thoracotomy was done at the 4th intercostal space and the upper left pulmonary veins were ligated. The chest were closed and fetus were returned to the uterus, the hysterotomy insision and the abdominal wall were repaired and subsequent gestation was allowed to proceed normally. Three ewes died of sepsis and pneumonia. Three pairs of twins were recognized during operation. One of twins was operated and another was used for control. All lambs died of unknown cause within 24 hours after spontaneous delivery. Both lung were removed and fixed in formalin. Several tissue blocks were taken from each lobe, embedded in paraffin, sectioned at 3 mum and stained with Goldner's trichrome combined with weiger's stain. Morphometoric analysis was performed employing the method that were developed by Suwa for measurement of the medial thickness and radius of pulmonary arteries and veins. The medial thickness of pulmonary vein and artery increased in the ligated lambs in comparison with the control lambs. However there was no significant difference between the ligated and unligated lobes. In fetal circulation, pulmonary flow is only 7% of combined right and left ventricular output. Our data suggests that pulmonary vascular change will occur even in fetus at the presence of pulmonary venous obstruction.
