Research Abstract

プロテインCは生理的に最も重要な血液凝固制御因子であり、血液中をセリンプロテアーゼ前駆体として循環している。一方、微小血管内で生成されたトロンビンは、フィブリン形成や血小板の活性化よりも速く血管壁内皮細胞上のトロンボモジュリンに結合し、プロテインCを特異的に活性化する。活性化プロテインC(APC)は、凝固Va因子およびVIIIa因子を分解・失活化し、凝固反応を強く阻害する。他方、APCは血液中のセリンプロテアーゼインヒビター(SERPIN)のプロテインCインヒビター(PCI)やα_1-アンチトリプシン(α_1-AT)と複合体を形成し、肝や腎の特異的レセプターと反応して血液中から排除される。本研究代表者らは、これまでにAPC・PCI複合体およびAPC・α_1-AT複合体について種々の基礎的研究を行い、APC・PCI複合体がこれまでに知られているいくつかの血液凝固亢進状態分子マーカーよりも早期にかつ特異的に血液中に出現し、その病態を反映して変動することを示唆してきた。本研究課題においては、本年度は、昨年度までに開発した凝固亢進分子マーカーのAPC・PCI複合体および類似のAPC・α_1-AT複合体に対する特異的酵素免疫学的測定法(ELISA)せ応用するため、多くの臨床検体の測定を中心とした研究を行った。その結果、播発性血管内凝固亢進状態(DIC)や血栓症などの血管内凝固の亢進が予想される疾患において両複合体の血中濃度が著しく増加し、またDICの治療にAPC製剤を用いた患者においても同様にAPC・PCI複合体濃度が特異的に増加していることが判明した。こうした結果から、APC・PCI複合体およびABC・α_1-AT複合体は新しい凝固亢進分子マーカーとして有用であることが臨床的にも証明できたと考えられる。最終年度にあたり、現在、本研究で得られた成果をとりまとめて複数の論文として報告するため準備を行っている。
