Research Abstract

布性人工血管に安定した内膜を形成させる方法として、高有孔性人工血管を使用しながらも出血の危険性を無くした方法を開発した。今日一般に臨床で布製人工血管を使用する際、布の網目を血栓で目詰まりさせる方法を採用している。しかしながら、この方法では術後に強力な抗疑固療法を行ったり、予期に反して線維素溶解現象が亢進した場合など、目詰まりしていた血栓が溶解し、急激な大量出血を来すという危険性があり、そのため網目の小さな低有孔性人工血管しか使用できなかった。そのため、人工血管内面には新生内膜が形成され難く、人工血管は長時間経過しても内面に血栓が付着した不安定状態であった。我々は自己の皮下脂肪組織を細切し、これらを用いて人工血管の網目を目詰まりさせることに成功した。この方法によると線維素溶解現象の影響を受けないため、それが亢進しても、強力な凝固療法を行っても出血しないと思われる。この仮説は動物実験により実証することができた。すなわち対照の血栓による目詰まり法では動物の体重当たり400単位のヘパリンを投与すると、止血困難な持続する出血を認めたが、我々の方法では1400単位投与しても出血はみられなかった。また、胸部大動脈への植え込みでは、対照群では20%に出血死をみたが、我々の方法ではそれは皆無であった。一方、人工血管内面での内膜形成期間は対照では7カ月以上必要であったが我々の方法では1カ月で完成し、人工血管は天然の抗血栓性を持つ内皮細胞で完全に内面は覆われ、それ以後の血栓付着はみられなかった。従って我々の方法では、結果的に、布の枠組みとして、これに自己の組織が付着し、最終的に体内での組織培養のような形で新しい血管壁を形成させることができた。
