Research Abstract |
The effect of local factors on nifedipine-induced gingival overgrowth is not clear. We demonstrated moderate gingival overgrowth during daily long term nifedipine treatment (3mg/kg) which had not major influence on blood pressure of a dog after 25-week in the right jaws with established gingivitis but did not in the left with a healthy gingiva subjected to toothbrushing twice a week and scaling once a week in a pilot study. Canines on both sides of the upper were used in additional 4 dogs (3 dogs in experimental and one in control). A ligature was placed around neck of the canine on the left sides only before given nifedipine. Nifedipine was given placed in soft food daily for 18 week, 6mg/kg in 2 dogs and 4.4mg/kg in one dog, but nifedipine was not given in a control dog. After given nifedipine, on left side in 2 dogs toothbrushing and scaling were continued, but in a dog was not for 18 week. Gingival size change on buccal side in coronal direction was estimated on stone casts using calipers from gingival margin at mesial, medial and distal site to coronally reference notch and changes in bucco-lingual and mesio-distal one were on the standadized photograph taken from the occlusal surface of canine before given nifedipine and after 18-week. In experimental group, coronal gingival change was detected in one site of the 9 examined, mesio-distal change was in 2 site (one in the right and other in the left) of the 3 examined, and buccal gingival change was in all sites of the 3 examined, but in control, gingival changes in any directions were not detected. These results suggest that it may be necessary to continue local irritant such as wire ligature during 18 weeks to establish experimental model for studying the mechanism of this side effect.
