Research Abstract |
The molecules of the bilirubin are said to be diverted to the photoisomers with the light. These photobilirubin (EZ,ZE-bilirubin) are water-solube and likely to be excreted from the liver and kidney. So we hypothesized that the phototherapy (PT) for the hyperbilirubinemia (HB) of neonates could also be indicated for the treatment of HB of an adult using a new method of direct irradiation of the light on the circulating blood. This study represents the results of the experiments. Experimental Protocol ; Eight pigs were anesthetized and artificially ventilated throught tracheotomy. The common bile duct was ligated and inserted with a cannula for sampling. The buffered bilirubin solution (unconjugated bilirubin powder, 0.1N NaOH,5% porcine albumin, 0.055 M sodium phosphate buffer) was infused at a given rate. ave was 510 nm was continuously irradiated for 12hrs (PT- group, n=4). The other four pigs were not irradiated during the procedure (CTR group, n=4). These parameters were assessed : total bilirubin (TB), direct bilirubin (DB), indirect bilirubin (IB), and alkaline phosphatase (ATP) in the serum, urine, and bile. Results ; (1) The excretion rate of the TB into the bile was significantly higher in the PT group than that in the CTR group three hours later from the initiation of the irradiation (p<0.05). (2) The excretion of the TB in the urine rapidly increased and maintained at a higher level in the PT group, in contrast to that of the CTR group (p<0.05). (3) The excretion of IB into the bile decreased in the CTR group, though that of PT group didn't change. (4) In the PT group, we found the marked increase of IB in the urine one hour later from the intiation of the irradiation, and there was a significant differences (p<0.05). Conclusion ; The phototherapy with direct irradiation on the circulating blood is so efficient that it is likely tobe used for the treatment of hyperbilirubinemia of an adult due to many other disorders.
