Research Abstract

「目的」われわれはこれまでGuillain-Barre Syndrome(GBS)患者から分離されたPenner19型のC.jejuniのリポ多糖とGMIガングリオシドの分子相同性を証明したが、本研究ではGBS患者から分離されたPanner2型のC.jejuniのリポ多糖とガングリオシドの分子相同性を明らかにし、GBS発症において病原体と神経の分子相同性がconventionalであることを示す。また、GBSの亜型であり抗GQ1b抗体の上昇するFischer症候群から分離されたC.jejuniのリポ多糖を分析し、ほかの疾患においてもその発症において病原体と神経の分子相同性の存在が重要であることを明らかにする。
「結果」C.jejuniが分離されたFischer症候群患者2例の抗ガングリオシド抗体を検討した。その結果2例とも抗GQ1b抗体が陽性であり、抗体価は経過とともに低下して行った。モノクローナル抗GQ1b抗体を用いて、分離されたpenner2型のC.jejuniのリポ多糖にこの抗体と反応する成分があることを明らかにした。
以上の結果からFischer症候群から分離されたC.jejuniのリポ多糖がGQ1b様構造を有することを明らかになり、これが宿主に抗GQ1b抗体を産生させ、抗体が神経筋接合部に作用することによって神経障害を惹き起こさせると考えられた。本研究からGBS疾患以外の患者においても病原体と神経構成分子との分子相同性が証明されその普遍性が示唆された。
