Research Abstract

発症予防や治療のないATLは,HTLV-1の感染予防が唯一の予防方法である.HTLV-1の地域内流行をコントロールするための適切な感染予防方法を確立し,将来のATL・HAM発症を防止することを研究の目的とする.
1.前方視調査による長期母乳哺育群のHTLV-1母子感染率は,15〜20%研究開始直後の予想と一致した.長期母乳哺育で感染させた母親の抗env抗体価は,させなかった母親より有意に高かった.これは,血中,おそらくは母乳中のプロウイルス濃度に依存する.
2.人工栄養児の感染率は約3%で,母子感染の約80%以上が遮断された.人工哺育で感染させた母親の抗env抗体価は,させなかった母親より有意に低く,母親からの移行抗体が感染予防に役立っていると思われる.
3.短期母乳哺育は,感染率を下げる.しかしながら,応用方法についてはなお問題がある.
4.人工栄養児の母子感染経路は,子宮内感染が主役でない.移行抗体の関与があるので,人工栄養児への感染も出産時,または出産後の感染と考えられる.
5.移行抗体の感染予防効果の証明により,抗体やワクチン投与により感染予防の可能性がある.抗HLTV-1 IgGやワクチニアワクチンは,動物実験で有効であった.
6.ウイルス負荷量と抗体価の間の強い相関,ウイルス負荷量と抗体出現の相関等,今後キャリア体内のHTLV-1の動態を理解する上での興味深い知見を得た.
HTLV-1の感染防止は,日本でのみ可能な課題である.HTLV-1の地域内流行の将来を見守るには,フィールドを潰すことなく,今後も長期間の追跡調査を続ける必要がある.
