Research Abstract

Model experiment
We evaluate the controllability of five flow control valves (Sophy, Medos, MDM,Orbis-Sigma, and Delta valve). Only delta valve showed different controllability by an implantation site. The Orbis-Sigma valve showed the lowest flow and both the Sophy and Medos valve showed highest flow. The MDM valve situated between these. Valve resistance of programmable valves (Sophy, Medos and MDM valve) is changeable within less than 40% of total systemic resistance, which showed narrow control range of these programmable valves.
Clinical investigation
We investigate postural changes in intracranial pressure (ICP), intra-abdominal pressure (IAP) and shunt flow. In sitting position, ICP decreased about 18 cmH2O and IAP increased about 22 cmH2O,both of which acted to decrease the siphoning effect. As a result, the siphoning effect decreased to about one third of the hydrostatic pressure. ICP and shunt flow became constant within 20 minutes after postural change from the supine to the sitting position.
Future aspect
We are going to measure in vivo shunt flow by developing a shunt flow meter. These data of ICP and shunt flow in vivo will be very useful not only in clarifying the pathophysiology of hydrocephalus but also in computer simulation of V-P shunt system. We want to establish the ideal shunt system through these investigation.
