Research Abstract |
The MR and dynamic MR images of 160 patients with soft-tissue masses were examined. In 122 patients, the diagnosis was proved by analysis of specimens. We correlated in detail MR findings with histopathologic findings in 18 tumors. We evaluated prospectively the differential diagnosis in recent 55 patients. We could speculate a lot of components (ex. : fat, fatty necrosis, cyst, cystic necrosis, myxomatous component, cartilage, collagen, hemorrhage, hemorrhagic necrosis, coagulated necrosis and bean-cured refuse like component) and the vascularity of soft-tissue mass by the MR and dynamic MR images in almost all patients. The composition and distribution of the components and the clinical findings could make the appropriate differential diagnosis easy. Especially, the specific diagnosis of lipmatous tumors, hemangiomatous neoplasms, peripheral nerve tumors, and myxoid tumors could become posible in a lot of cases. In 115 patients, the dynamic MRI pattern was help in distinguishing benign from malignant soft-tissue tumors. But it was imposible to distinguish perfectly benign from low grade malignant tumors, and active inflammatory from malignant masses. The change of size, internal structure, and the slope of the dynamic MRI pattern was helpful to assess the appropriate responses to chemotherapy and irradiation. Dynamic 3D Gd-enhanced MR angiography was a new effective technique providing appropppriate essential vascular anatomic information and tumor vascularity prior to surgery, and for assessing the responses to chemotherapy and irradiation.
