Research Abstract |
Elevation of the incompatible antierythrocyte antibody (hemagglutinin) titer, in the postoperative period, was detected in the 3 patients of 4 ABO incompatible living related liver transplantation (LRLT) cases (75%). But an increase in incompatible hemagglutinin titer didn't always lead to acute rejection and wasn't relatet to level of compatible hemagglutinin titer. In our series of 6 identical and compatible LRLT,elevation of the compatible hemagglutinin titer was detected in the 4 cases (67%) and there wasn't significant difference blood loss, transfusion, episode of infectious complication and acute rejection, duration of anhepaticphase, and protocol of immunosuppression, between elevated and non-elevated cases. There were 4 postoperative cases in 13 pediatric surgery (31%) and 3 in 24 general surgery (13%), respectively, with hemagglutinin titer increasing. These results suggest that some factors, stimulating production of natural occurring antibody, is present. In 2 successful cases of ABO incompatible LRLT,incompatible hemagglutinin titer has been low grade, although compatible hemagglutinin titer had reached to preoperative level, during long term postoperative course. This fact may signify that there are some factors by which the synthesis of only incompatible hemagglutinin is suppressed. Though It's reported that there is almost no hemagglutinin in the plasma of neonate, according to our study, in 28 normal neonate, 3 cases had apparently high hemagglutinin titer. And, hemagglutinin titer is on the increase by age and difference among individuals was large.
