Research Abstract |
A cohort study on osteoporosis was performed in two communities with different characteristics in Wakayama prefecture in order to determine the distribution of bone mineral density (BMD) and the incidence of fast bone losers of lumber BMD by sex and age, and also to clarify factors affecting changes in BMD.In Miyama village, located in a mountainous area, a cohort of 1543 inhabitants aged 40-79 years (716 males and 827 females) was established from a register of inhabitants. In Taiji town, located in a seaside area, another cohort of 2261 inhabitants aged 40-79 years (1028 males and 1233 females) was identified. Fifty men and 50 women in each age group between 40 and 79 years, a total of 400 participants, were recruited randomly. Initial examination of lumber BMD was carried out in 1990 in Miyama and in 1993 in Taiji. After 3 years, a follow-up study was performed in each area on the same subjects. in both the initial and follow-up studies. The BMD was higher in males than in females in any age group and an age-related decrease in BMD was observed in both areas. The BMD was significantly higher in the seasside population compared with mountain population in both sexes in any age group except males in their 50s. Changes in mean lumber BMD during 3-year period were compared by sex and age. In men, almost no rate of change in BMD was observed. By contrast, in women, a decrease in BMD was observed. The incidence of fast bone losers was 2.4% in men and 9.7% in women in Miyama village ; in Taiji town, it was 2.2% in men and 6.9% in women. Factors affecting annual changes in BMD were also investigated with a cohort study and analyzed by multiple regression analysis. The following items were significantly related to annual changes in BMD : medication with tranquillizer in both sexes, milk consumption in males, body weight and height at the initial measurement, and small fish consumption in females.
