Research Abstract |
Objection : Cry opreserved allografts have been utilized as alternatives for aortic and pulmonary valvular prosthesis with satisfactory results. However, allogarfts in the mitral position has not been popular because of dificulty in placing mitral allograft in proper position due to complexed anatomy of subvalvular apparatus. We have, therefore, investigated the feasibility of freehand cryopreserved allograft in the mitral position in pigs. Methods : Hole cry op reserved mitral valvular allografts with papillary muscle, chordae, and leaflets from donor pigs were implanted to the mitral position of recipient pigs under cardiopolmonary bypass. Post-implantation hemodynamics and valvular function were evaluated by measuring arterial pressure (AP), left ventricular end diastoric pressure (LVEDP), and left atrial pressure (LAP) and by evalurating two-dimensional echocardiography. Pathohistological evaluation of allografts after cry opreservation and operation were performed by light microscopy. Results : Hemodynamic data were shown as follows : AP ; 124*6mmHg, LVEDP 7*1 mmHg, LAP ; 8*2mmHg. Echocardiography showed the satisfactory opening of the mitral valve in diastole and the good leaflet coaptation in systole. Light microscopic examination of allografts after cry opreservation and operation showed almost normal structure. Adventital inflammatory change in the papillary muscle after operation was not observed. Conclusions : Acute hemodynamic function of freehand cryopreserved allografts replacement in the mitral position in pig was acceptable.
