Research Abstract |
WE INVESTIGATED THE CORRELATION BETWEEN SEPTIC PULMONARY INJURY,WHICH IS INDUCED BY EXCESS INFLAMMATORY RESPONSE,AND ORGAN/PERIPHERAL CYTOKINES (IL-6, IL-8, IL-10) FOR THESE 2 YEARS. (1) USEFULNESS OF ORDINARY BLIND-SUCTIONED SPUTA AS SPECIMEN FOR CYTOKINE MEASUREMENT (a) SPUTUM/BLOOD CONCENTRATION RATIOS WERE TNFalpha : ABOUT 80, IL-6 : ABOUT 800, IL-8 : ABOUT 1500, RESPECTIVELY IN PATIENTS WITH EXCESS INFLAMMATORY RESPONSE. (b) SERIES OF BALF SAMPLES : CYTOKINE CONCETRATION IN 1ST SPECIMEN WAS THE HIGHEST AMONG ALL SPECIMENS.THEREFORE,ONLY 1ST BALF SPECIMEN SHOULD BE USED FOR THE DETERMINATION OF CYTOKINE CONCENTRATION. (2) ESTIMATION OF PROGNOSIS BY THE BLOOD LEVEL OF IL-10 : PATIENTS WITH LOW BLOOD LEVEL OF IL-10 AND HIGH LEVELS OF IL-6 AND/OR IL-8 WERE SUCCESSFULLY TREATED.BUT,PATIENS WITH HIGH BLOOD LEVEL OF IL-10 HAD POOR PROGNOSES.THE ROLE OF IL-10, ANTI-INFLAMMATORY CYTOKINE,IS,THEREFORE,SEEMED COMPLICATED. (3) CORRECTION WITH BUN : BUN IS THOUGHT TO BE DISTRIBUTED UNIFORMLY IN THE BODY.THEREFORE,IT IS REASONABLE TO CORRECT THE CONCENTRATIONS OF CYTOKINES IN SPUTA OR BALF WITH THE RATIO OF BLOOD/SAMPLE BUN CONCENTRATION.
