Research Abstract

To evaluate the utility of Peptide-EIA to detect HIV subtype (clade) cliniccaly isolated in Japan. With the use of EIA typing, 27% of our samples ware clade E. Most of them were male hetero-sexually infected. These results indicate the trend of HIV infection in Japan has been changing gradually. To better understanding the mechanis of long-term non-progressors (LTNP), we attempted following experiments. First we measured number of HIV-RNA by branched-DNA assay and of chemokine RANTES (acronym for Regulated upon Activation, Normal T cell Expressed and presumably Secreted) that reported to suppress HIV infection by ELISA by using 54 plasma samples that were HIV-1 seropositive including 7 of LTNP that were asymptotic carriers and kept 500 CD4 positive cells/ul and for 10 years or more. The quantitative assay of HIV-RNA could not detect (detection level, 10,000 Eq/ml) from plasma of LTNP, while, this quantitative assay showed HIV-RNA positive in the non-LTNP group. And in overtime analysis of HIV-RNA, this assay showed that kinetics pattern of HIV-RNA value in plasma of non-LTNP group could be classified to 4 different types as following, 1) increasing, 2) decreasing, 3) staying low level, 4) keeping high level. We also demonstrated that a case of asymptotic carrier who kept high level HIV-RNA value in plasma progressed to AIDS only 8 months later. By the RANTES analysis, we showed that its value in plasma of LTNP were obviously higher than those of non-LTNP group, and that in the samples in asymptotic carrier as stage, the group from which we could isolate HIV in culture showed significantly lower RANTES value in plasma than those of negative group. Finally, we compared cDNA sequence of PND region in gp120 by polyclonal analysis by using these isolated viral stocks of LTNP and non-LTNP group. The variation of sequence in LTNP group was little tor long term as 10 years, while it was not little in non-LTNP.
