Research Abstract

Purpose. The purpose of this study was to evaluate the ability of power Doppler sonography in differentiating metastatic from non-metastatic cervical lymph nodes in cancer patients.
Methods. Power Doppler sonography was performed on histologically proven 71 metastatic lymph nodes and 220 non-metastatic lymph nodes in 77 patients with head and neck cancer. Power Doppler sonography was evaluated to determine its ability to differentiate metastatic from non-metastatic lymph nodes in comparison with the decision made on CT (the presence of central necrosis or ring enhancement) or with the size determination (transverse to longitudinal ratio, T/L ratio) of lymph nodes on sonography.
Results. Power Doppler sonography showed characteristic features of power Doppler signals of the blood flow in the parenchyma of the metastatic lymph nodes. In addition, power Doppler sonography showed high levels of sensitivity (82%) and specificity (97%) in depicting metastatic lymph nodes, which was superior or comparable to those by CT (53% sensitivity and 100% specificity) and by T/L ratio (64% sensitivity and 87% specificity). A combination of the sonographical criteria (parencymal color signal or T/L ratio) improved the diagnostic accuracy (91% sensitivity and 100% specificity).
Conclusions. Taken together, these results suggest that the power Doppler sonography could be a powerful tool for depicting metastatic lymph nodes in cancer patients.
