Research Abstract |
Matched case control studies on gastric cancer were carried out in Guatemala and Costa Rica, in order to c1arify the risk factors of gastric cancer by histologic type (Diffuse type (DT) and Intestinal type (IT)). Cases were histologically confirmed gastric cancer patients in hospitals. Controls were recruited from admitted patients, matched by sex, age (*3 years) and hospital. Life style, dietary habits were obtained by interview using questionnaire. Blood samples and pathological specimen were collected. Serum were analyzed for H.pylori antibody and pepsinogen in Japan. Statistical analysis was done by Cox proportional hazard model by SAS. In Guatemala, among 147 cases and 176 matched controls, 55 pairs of DT and 82 pairs of IT were analyzed. It was observed that RR differences between DT and IT were detected in refrigerator use and low income. For H.pylori, no RR increase was observed both in DT and IT gastric cancer. For pepsinogen, RR for IPGI / II <3.0in IT was higher than DT pairs. In Costa Rica, among 173 cases and 183 matched controls, 61 pairs of DT and 93 pairs of IT were analyzed. In DT cases, no particular life styles showed high or low RR.But, in IT cases, RR of refrigerator use was high. RRs of H.pylori were not increased both in DT and IT gastric cancer. Also, for H.pylori no RR differences between DT and iT was observed. For pepsinogen, RRs of PG 1/II<3.0 were higher both in DT and IT.But no clear differences was observed between DT and IT. In summary, this study may suggest that IT and DT gastric cancer has different etiology, although further data collection should be required.
