| Research Abstract |
Ventricular tachycardia and fibrillation (VT/VF) are serious problems in the patients with organic heart disease. However, it is not easy to evaluate risk of VT/VF because of methodology. We focused on noninvasive methods especially ECG T wave alternans (TWA) and designed a prospective study of VT/VF in the patients with organic heart diseases. About 250 patients were examined. The first pilot study was designed to test the methodologies in the various heart disease including previous myocardial infarction, cardiomyopathy, hypertensive LV hypertrophy and normal volunteers. TWA is closely correlated VT/VF in the patients with DCM, but not in the patients with HCM or MI. The positive criteria should be changed depending on the heart diseases. The second pilot study was done to test whether TWA was related to VT/VF in the patients with congestive heart failure. VT/VF risk is very high in the patients with TWA positive, LP positive and low LV function. The third pilot study was done to test whether TWA is correlated histological damage in the patients with LV hypertrophy. TWA is more frequently positive in the patients with HCM and sever histological findings than in the patients with hypertensive LV hypertrophy. Thus, TWA may be related to histological damage of myocardium. A prospective study is on going to test TWA as a maker of VT/VF risk in the patients with organic heart diseases. Briefly, patients with sustained VT showed very poor prognosis, but patients with non-sustained VT and without VT showed relatively good prognosis, despite nearly identical LV function.
