Research Abstract |
To clarify relationship between angiogenesis and growth and spread in gynecologic malignancy, we investigated how MVD is related with several clinicopathological parameters and correlated with metastasis and survival in endometrial carcinoma and adenocarcinoma of the uterine cervix. All hysterectomy specimens were stained immunohistologically for factor VLII-related antigen. MVD was counted in a x200 field (0.785 mm2 per field) in the most active of neovascularization. In endometriaL carcinoma (85 cases), MVD was significantly correlated with tumor grade, myometrial invasion, and lymph-vascular space invasion. Patients with high MVD (more than 60) had significantly worse PFS and OS than those with low MVD (less than 60). Multivariate analysis showed that MYD correlated significantly and independently with PFS and OS, and MVD is an independent prognostic factor for PFS and OS. o examined. Twenty-six of 70 (37%) had hyperplasia in the endometrium adjacent to the carcinoma. Significantly more patients with low MVD (less than 60) were in carcinoma with hyperplasia compared to carcinoma without hyperplasia. Significantly more patients with p53 expresssion were in carcinoma without hyperplasia compared to carcinoma with hyperplasia. The presence or absence of hyperplasia seems to be important to assess the biological behavior of endometrial carcinoma, especially concerning angiogenesis and p53 expression. In adenocarcinoma of the uterine cervix, MVD were significantly increased in invasive area as compared with adjacent non-neoplastic area. MVD was also significantly correlated with ascites cytology. Patients with high MVD (more than 75) had significantly worse PFS and OS than those with low MVD (less than 75).
