性ステロイド依存性腫瘍におけるestradiol局所調節機構の解析

Research Abstract

平成10年度は、ヒト乳癌(90例)及び子宮内膜癌組織(35例)における17β-hydroxysteroid dehydrogenase(17β-HSD)isozymesの発現を免疫組織化学的に検討した。その主な結果は下記の如くである。
1。 浸潤性乳管癌では、59%の症例で腫瘍細胞に17β-HSD type 1の発現が見られたが、17β-HSD type 2の発現は認められなかった。
2。 17β-HSD type 1の発現が高い群ほど、腫瘍の形態学的分化度がよく、estrogen receptorやprogesterone receptorが多く発現し、またKi-67増殖関連抗原の発現が低い傾向が統計学的に示された。
3。 子宮類内膜腺癌では37%の症例において、腫瘍細胞に17β-HSD type 2の発現が見られたが、17β-HSD type 1の発現は見られなかった。
4。 腫瘍の形態学的分化度の良い症例ほど17β-HSD type 2の発現が高頻度で認められた。
ヒト癌組織における17β-HSD isozymesの発現パターンは、平成9年度のin vitroの実験データと類似し、乳癌と子宮内膜癌では大きく異なっていた。すなはち癌組織におけるestradiolは、乳癌では合成によって制御されるものの、子宮内膜癌では逆に分解によって制御されていると推察された。以上の所見は、乳癌や子宮内膜癌におけるestradiolの局所的役割や腫瘍の増殖形態について考えるうえで大変重要な所見と思われた。
