Research Abstract

HIV-1感染無症候キャリアー(AC)の末梢血CD8陽性T細胞はHIV-1抑制活性を持ち、複数の機序によるこの活性がAC期のウイルス量の低下に貢献していると考えられている。我々はそれぞれの機序のHIV-1抑制活性と病態の進行との関連を明らかにするために、まずMHC-I拘束性の有無によって抑制活性を区別して検出する系を作出し、HIV-1感染の各ステージにおけるこれらの活性の差異について解析を行った。
はじめにCD4陽性PBMCに試験管内でHIV-1を感染させ、AC由来のCD8陽性細胞と共培養したところ、HIV-1産生量の低下がみられた。この抑制は標的が自己CD4陽性細胞である場合に強い傾向があったが、全くMHC-Iを共有しない場合にも有効であった。一方、HTLV-I感染細胞株ILT-KKにHIV-1を感染させて同様の検討を行ったところ、MHC-Iが共有する場合のみに抑制が認められた。即ち、PBMCに有効であったMHC-I非拘束性の抑制はILT-KK細胞には無効であることが判明し、MHC-I非拘束性と拘束性の抑制が、MHC-Iの一致しないPBMCとILT-KK細胞をそれぞれ標的に使えば区別できることが示された。次にこの系を用いて種々のステージのHIV-1感染者を調べたところ、末梢CD4陽性細胞数が200/μl川以上の患者ではMHC-I拘束性、非拘束性の両活性とも認められたが、CD4数50/μl以下の患者では両者ともに減弱していた。興味深いことにAIDS発症への移行期にある患者(CD4数50〜200/μl)では、MHC-I非拘束性の抑制のみが減弱していた。AIDS期にはCTL活性が消失することが知られているが、本研究により、AC期にMHC-I拘束性CTL活性と非拘束性抑制活性が保たれている一方で、移行期において後者が先行して減弱する可能性が新たに示された。
