Research Abstract

子宮平滑筋はプロスタグランジン(PG)により収縮し、一酸化窒素(NO)-可溶型グアニル酸シクラーゼ(sGC)-cGMP系の活性化により弛緩することが知られている。本研究では、PG産生の律速酵素であるcyclooxgenase-2(COX-2)、PG受容体、およびNO-cGMP系の妊娠子宮筋における発現を解析し、早産治療法開発の可能性を開くことを目的とした。今年度の結果は以下のとおりである。
1) PG受容体遺伝子発現の検討では、子宮収縮作用のあるEP3受容体の遺伝子発現は、妊娠初期・後期の子宮筋では非妊娠子宮筋の発現の約60%に減少した(P<0.05)が、平滑筋弛緩作用のあるEP2およびEP4受容体遺伝子発現は少くかつ妊娠では不変であった。
2) Nor ther n bl otによる検討では、弛緩出血症例の子宮筋のEP3遺伝子発現は正常であったが、FP受容体の遺伝子発現異常の存在が示唆された。
3) 経腟分娩(陣痛発来後)例の羊膜のCaX-2蛋白発現は、子宮頚部側(100%)>反対側(39%)>胎盤表面(21%)であり、頚部側で有意に発現が亢進していた(P<0.05)。しかし、陣痛発来前の予定帝王切開例の羊膜組織ではCOX-2蛋白の発現はどの部位でもごく少量であった。
4) NOにより活性化されるsGCの子宮体部組織における活性(cGMP産生能)は、妊娠中期子宮筋では平均142pmol/10min/gあったが、妊娠末期には、陣痛発来前、陣痛発来後いずれの群でも有意に減少した(P<0.05)
5) 妊娠中期子宮筋のcGMP含有量(137pmol/g)は非妊娠時(33pmol/g)より高かった(p<0.05)。
6) 妊娠末期陣痛発来前の子宮筋のcGMP含有量は70pmol/gであり、妊娠中期に比して有意に減少しており(P<0.05)、陣痛発来後の子宮筋では47pmol/gとさらに減少した。
