Research Abstract

本研究では浮腫大脳の冠状断面の弾性率分布と浮腫の病理組織像との対比をおこなった。浮腫の脳内の分布はMRIを用い、水分増加量の指標とじてT2値を、細胞内水分増加量の指標として拡散係数値を求めた.
実験にはネコを用い、虚血性脳浮腫(中大脳動脈閉塞)と外傷性脳浮腫(凍結脳損傷)を発症させた.摘出脳冠状断切片の圧迫による変位をスキャンし、弾性率は陥凹の程度と相関するため、弾性率の分布を陥凹の程度から求めた.
浮腫脳の各部位から小サンプル(10mm3)を切り出し組織比重法により浮腫の程度(水分含量の増加の程度)を求め、水分含量と弾性率が灰白質や白質でどの様な相関を示すか検討した.また潅流固定後の浮腫脳を電顕的に検討し浮腫液の局在や細胞の変化と拡散係数の変化とどの様な関連がみられるか検討した.
その結果、以下の点が明らかになった.
1) 組織の弾性特性は血管原性脳浮腫と細胞障害性脳浮腫では水分量増加との相関が逆の傾向を示した.即ち血管原性脳浮腫では水分量の増加とともに弾性は低下し、細胞障害性浮腫では弾性率は上昇した.
2) 血管原性脳浮腫では水分量の増加とともに組織水の拡散係数は上昇するのに対し細胞障害性脳浮腫では水分量の増加とともに拡散係数は弾性は低下した.
3) 血管原性脳浮腫では浮腫液は細胞外腔に局在し細胞障害性脳浮腫では浮腫液はアストログリアなどの細胞内に局在した.
