Research Abstract

ヒトPMFは,発症早期の著明な骨髄線維化,多臓器での髄外造血と巨脾,体重減少と貧血などを呈し,死亡率が高い.真性多血症(PV),本態性血小板増多症(ET)は類縁疾患であるが,前述のような特徴は認めず予後もよい.つまりJAK2-V617Fという同一の遺伝子変異を持つにもかかわらず,PMFとPV,ETの間には病態に明らかな差異がある.
我々は作成したモデルマウスにヒトPMFとPV,ETとの間で観察される病態の差異が再現されていることに着目し,モデルマウスの病態差異の分子機構を解析した.野生型マウス,PMFの病態を呈するTGマウス,PV,ETの病態を呈するTGマウスの3群の造血幹細胞の遺伝子発現をアレイによって比較し、PMF病態を呈するマウスに特異的に変動している遺伝子群を抽出した.抽出された複数の転写因子についてin vitro,in vivoの機能解析を行い病態との関わりを検討した。
PMFに対する新規治療法として現時点で最も有望であるJAK2阻害剤について、その前臨床研究を行った.R723およびNS-018はin vitroでJAK2に対して強力で選択性の高い抑制効果を示す新規JAK2阻害剤である.ヒトPMF類似の病態を呈するTGを用いてその治療効果と作用メカニズムを検討した.R723およびNS-018はJAK2-V617F変異に起因するマウスPMFの血球増多,髄外造血といった病態を改善し,ヒトPMFに対するJAK2阻害剤として期待される薬剤であることを示した。
骨髄増殖性疾患を始めとする骨髄系腫瘍で広く変異が認められるTET2遺伝子について、マウスを用いin vivoの機能を明らかにした。
