Research Abstract |
Aim : The study focused on the analysis of periprosthetic bone remodeling 1) after morselized allograft bone by impaction in rat femur model, and 2) periprosthetic bone around loose total hip arthroplasty and status after impacted morselized allobone in revision surgery. Materials and methods : Morselized allograft bone were impacted with stainless pin in 12 week old rat femur model. The femurs were resected and fixed with 70% ethanol solution. on 1, 2, 4, and 8 weeks after surgery. Prior to resection of the femurs, tetracycline hydrochloride was loaded two times for bone labeling. After Villanueva bone staining, the femurs were embedded into methylmethacrylate and sectioned for histological analysis by fluorescent microscopy. same as 1). Results : 1) Periprosthetic bone remodeling was observed in the morselized allobone adjacet to cortex, and proceeded to the implants gradually. Complete bone remodeling was not observed after 8 weeks of the impaction. 2) Increase of tetracycline-labeled bone surface, osteoid/immature calcified area, and osteoblast lining were the significant findings in the periprosthetic bones adjacent to loose interface. Osteoclasts and resorption lacunae were also abundantly observed. The presence of tetracycline deposition, osteoid, and immature calcified bone matrix was markedly observed adjacent to monocyte/macrophage cell sheets in the granulomatous tissues. In the retrieved impacted allograft bone, mature bone remodeling was observed near the cortex, but which was insufficient around implants. Conclusion : High turnover bone remodeling and insufficient bone formation exist around loose implants, and contribute to the periprosthetic weakening and osteolysis. Once morselized bone was impacted into periprosthetic area, bone remodeling has occur adjacemnt to cortex at initially, and proceed gradually to implants.
