Research Abstract

3 courses of two route intra-arterial chemotherapy using cisplatin (100mg/mィイD12ィエD1) and sodium thiosulfate on day 1 and a 5-fluorouracil (5-FU) dose of 1000mg/mィイD12ィエD1) 24-hour infusion on day 2-6 were given in 100 patients with untreated head and neck squamous cell carcinoma. The last third chemotherapy combined with radiation therapy with a dose 60-70Gy. Original site of cancer was the maxillary sinus in 20, nasopharynn in 14, mesopharynx in 23, larynx in 4 and oral cavity in 39 patients. The clinical stage was stage I in 1, state II in 17, stage III in 36 and stage IV in 47 patients. The endpoint of this trial is the survival and organ preservation.
The three year survival rates were 69% in maxillary sinus, 90% in nasopharynx, 74% in mesopharynx and 62.5% in oral cavity. The organ preservation rates were 56% (9/16) in maxillary sinus, 65% (15/23) in mesopharynx and 39% (13/33) in oral cavity.
We concluded that the conbination of selective delivery of cisplatin/intra-venous continuous infusion of 5-FU and comcomitant radiotherapy can be safely and effectively applied to patients with head and neck squamous cell carcinoma. Preliminary complete response indicates that this approach may improve the organ preservation for patients with maxillary sinus, mesopharynx and larynx.
