Research Abstract

Sulfatide reacting strongly with L- and P-selection plays an important role in the initiation of neutrophil-endothelial interactions occurring in injured or inflamed tissues. We have already studied that sulfatide has a significant protective effect against ischemia-reperfusion injury. There are many reports that neutrophils affect the unpredictable distal flap necrosis. The purpose of this study was to evaluate the effect of sulfatide on survival of random pattern flap in rat.
We used the cranially based dorsal skin flaps elevated with the panniculus carnosus measuring 15×60mm. The control flaps (n=10) had a mean of survival length of 41.5±2.1 mm compared with 49.5±1.7 mm for sulfatide treated (n=10) flaps (p<0.01). The significant difference existed between a mean of survival length (49.1±2.0 mm) and a mean of dye distance (39.7±1.1 mm) in treatment (n=10) group (p<0.01), while no significant difference in control group (n=10). In contrast that there was no significant difference in control group (n=5) in comparison with each values of the means of dye distance measured with the lapse of time and the means of survival length, the significant differences (p<0.05) existed between the value at one hours and the rest values and between the value at 24 hours and the value at 48 hours in treatment group (n=5). The remarkable leukocyte infiltration in the dermal layer appeared after 48 hours postoperatively in control group, but didn't in treatment group histlogically.
We concluded sulfatide is able to rescue distal flap necrosis dependent on hemodynamic, cellular, and coagulation mechanisms. Moreover we consider the important period to clarify the dramatical mechanism of the effect of sulfatide against the distal flap necrosis is till 48 hours especially after 24 hours postoperatively.
