Research Abstract |
Acute oral poisoning with an berbicide BASTA^<【○!R】> causes consciousness disturbances, convulsions, and apnea after 6-40 hours of ingestion. Because, the patients look non-severe before the onset of these signs, the respiratory management might be late for sever patients. On the other hand, unnecessary intensive treatments such as hemopurifications are sometimes conducted for non-sever patients. For conducting a suitable management for this poisoning, by estimating the severity of this poisoning correctly before the onset of severe signs, we studied serum glufosinate concentration ([GLF]s)and amount ingested(AI)in relation to the severity of the patients. [Method] Ninety-nine cases of BASTA^<【○!R】> ingestion were studied. Fifty-one cases that produced at least one of the following 3 sings : coma, generalized convulsions, and apnea, were defined as severe. Forty-eight cases that produced none of these sings were defined as non-severe. In each case, [GLF]s on admission was measured and AI was estimated. [GLF]s was plotted semi-logarythmically related to the hours after ingestion. [Results][GLF]s of sever cases located higher than those of non-sever cases. All cases aboye the line connecting 210 ppm at 2 hours and 15 ppm at 8 houres were severe. On the contrary, all cases under the line connecting 80 ppm at 2 hours and 5 ppm at 8 hours were non-severe. Between these 2 lines, severe and non-severe cases existed half-and-half. The AI in severe cases were higher than those in non-severe cases(p<0.0001). However, sever cases and non-severe cases could not be discriminated clearly based on AI, because AI in both groups were widely distributed, overlapping with each other. [Conclusion][GLF]s on admission, which can be measured in about 3 hours by HPLC, is a valuable tool in predicting the severity of BASTA^<【○!R】> poisoning. (284 words)
