Research Abstract

腸管と腎のNaCI代謝を結ぶ生理活性物質の存在が以前から推測されていたが、この腸管性ナトリウム利尿因子のlつの候補として、uroguanylinが1995年にラットの腸管から発見された。uroguanylinは15アミノ酸よりなる酸性ペプチドで、腎尿細管と腸管の上皮細胞に存在するguanylate cyclase結合C型受容体(GC-C)を活性化し、細胞内cyclic GMPを増加させ、腸管からのNaCl排泄と腎臓でのNa利尿に作用する。
申請者らは、in situ hybrdizationと二重免疫染色により、uroguanylinは胃の内分泌細胞であるenterochromaffin-like細胞で産生され、腸でも内分泌細胞のenterochrmaffin細胞で産生されることを明らかにした。胃におけるuroguanylin産生は、ヒスタミン負荷で低下し、プロトンポンプ阻害薬で増加した。またGC-C受容体はenterochromaffin-like細胞と壁細胞に発現しており、胃におけるuroguanylinは胃酸分泌調節に関与していることを示した。さらにラットの腸管灌流系(vascular&luminal perfusion)を用いた実験により、経管NaCl負荷に対し、腸管側と血管側の両方にuroguanylinが分泌され、その量比は4:1であることを示した。形態学的にもuroguanylinが分泌されている像を確認した。uroguanylinは腸管では、Na濃度により生合成と分泌が調節され、Na利尿に作用する腸管性Na利尿因子であることを提示した。
