Research Abstract

Extravillous trophoblast(EVT)は、着床後速やかに母体脱落膜へと侵入し、妊娠維持機構の中で重要な役割を担うと考えられるが、その増殖とアポトーシス調節機構はいまだ明らかでない。そこで、妊娠各期胎盤EVTにおけるアポトーシス発現とFas/Fas ligandならびにBcl-2蛋白の発現態度を調べ、妊娠経過に伴うEVTのアポトーシス発現調節の一端を明らかにしようとした。患者の同意を得て、正常妊娠初期(6〜12週)、中期(16〜20週)、末期(37〜41週)胎盤でのEVTのアポトーシス発現を電子顕微鏡下に観察後、TUNEL法により各期胎盤EVTのアポトーシス陽性率を検討した。また、Bcl-2蛋白とFas/Fas ligand発現はABA法で免疫組織学的に検討した。各期胎盤EVTで散在的にアポトーシス発現が観察されたが、EVTのアポトーシス陽性率は妊娠初期で有意に高く、妊娠中期以降低下した。妊娠中期と末期の胎盤では、脱落膜浅部のEVTに比して深部EVTでアポトーシス陽性率は有意に高かった。Fas/Fas ligand発現は妊娠各期胎盤EVTで観察され、その発現レベルは脱落膜浅部EVTに比して深部EVTで高いことを認めた。他方、Bcl-2蛋白の発現レベルは、妊娠各期胎盤EVTで差はなかったが、脱落膜浅部のEVTに比し深部のEVTで低いことを認めた。妊娠各期を通して多核EVTでは、単核EVTに比してBcl-2蛋白発現が強く、アポトーシス陽性率は低値を示した。妊娠初期EVTでアポトーシス発現は有意に高かった。多核EVTならびに妊娠中期以降の脱落膜浅部EVTではBcl-2蛋白によるアポトーシス抑制機構の存在が示唆され、EVTの脱落膜深部への侵入とともにFas/Fas ligandを介してアポトーシスが誘導されることが推察された。
