Research Abstract

家族性アミロイドポリニューロパチーの原因はトランスサイレチン(TTR)の変異によることが知られているが、この疾患は壮年期に発症し、診断および保因者の検出に、簡便で信頼の出来る検査法が望まれる。被検血清に抗TTR抗血清を加え、免疫沈降物を得、それを質量分析することにより、1時間以内に10μlの被検血清で診断しうることを示した。宮崎医大、熊本大、信州大、Umea大、Boston大において追視され、当教室で発見したG101S,D38Aなど新変異6種を含む少なくとも19種類の変異が検出可能であることを証明されている。
生体内のTTRには様々なアイソフォームが存在するが、その中で80ダルトン高質量の成分が亜硫酸化TTRであることを証明し、これが高いアミロイド原性を持つのに対し無修飾のTTRにはアミロイド原性がないことをin vitroで示した。亜硫酸は食物・薬品中に防腐剤として使用されているだけでなく、一般的な大気汚染物質である。トランスサイレチン関連アミロイドーシスは何らかの環境因子が発症機序に関与しているとする報告も多く、今後さらなる外因性亜硫酸塩とアミロイドーシスの研究を進めていく必要がある。
老年性全身性アミロイドーシスの世界的第一人者である、スウェーデンのPer Westermark教授との共同研究により、アミロイド組織中に質量数の異なる複数の成分があることを発見し、それらの大部分が抗TTR抗体と反応することを見出した。現在、構造解析中である。
モリブデン補因子欠損症において、血清中亜硫酸化TTRの比率が変化していることを見出し、報告した。この結果、オーストリアのインスブルック大学子供病院より、様々な代謝異常症例59検体の分析依頼を受け、現在解析中である。
