Research Abstract

Venous flap is a useful surgical method for covering skin ulcer. However, it is not clear how to survive without artery. We thought that the period prior to the complete restoration of blood supply is critical for venous flap survival. To clarify the role of plasmatic imbibition, in this periods, numerous biochemical studies and enzyme-histochemical studies have been performed However, there is no study mentioned about cell metabolism by plasmatic imbibition. So we examined the plasmatic imbibition of venous flap using BrdU, which is actively incorporated into nuclei during S phase. We produced 15 musculocutaneous (MC) venous flaps, 15 skin grafts as positive control and 15 MC grafts as negative control on the dorum of F344 rats. Then, BrdU(150mg/Kg) was administrated intraperinonelly. Each five flaps or grafts were harvested 1,3,7 days after transplantation, were fixed with 10% par formaldehyde and were made into paraffin sections. Each section was examined by light microscopy. BrdU uptake was examined immunohistochemically using co focal laser scanning microscopy. BrdU uptake was seen in all the layers between epidermis and dermis in skin grafts, in under layer of panniculus canorous in MC grafts, and between middle of dermis and the panicles carouses in MC venous flaps during the experimental period. All the layer of skin grafts was survived. All the layer of MC grafts became necrosis at 7 days after transplantation. All the MC venous flaps survived partially between middle of dermis and the paniculus carnosus.
Survival layer correlated to the BrdU uptake layer one day after transplantation. This indicated that the destiny of graft survival had already been determined prior to the complete restoration of blood supply.Consequently, MC venous flaps extended the survival area by extension of plasmatic imbibition area compared to MC grafts.
