Research Abstract |
In this study of a double wake-up method (reported in 1996-1997 Scientific Research Grant Results), individuals became accustomed to enjoyable conditions matched to their personality : Under such conditions, persons requiring care were ready to get up at a designated time (as a conditioned reflex). were able to get up by themselves, were able to rise easily, and did not experience drops in blood pressure. The subjects of the preliminary research were cram school students (with permission of doctors responsible for the students' local area) on summer break. The subjects of the study proper were 6 persons requiring care and living in an elderly living facility (8 sequential days ; age, 74-90 years ; bedridden for 3-9 years ; required care level, 5-3 ; 3 suffering dementia). 1.On the first day of the study, an advance visit was made to the homes of the bedridden subjects (for information gathering, observation of disease, obtaining doctors' permission, obtaining willing participation, etc.

). Use of the double wake-up method was begun on the following day, and the time that patients spent sitting up was progressively extended. Although subjects felt some unease and nervousness from day 1 through day 3, they gradually began to smile and talk more as the study progressed. Beginning on day 3, at the time they got up, the subjects engaged in activities they were good at and which they enjoyed. Emotional stimulation can be a strong motivator of physical activity, and each day's getting-up procedure should involve an activity which specifically suits the subject (singing a favorite song, listening to music, meeting grand-children or pets, speaking kind words, etc.). 2 Of the 6 subjects (each of whom had received 3 - 9 years of clinical care), 3 who were unable to get up before the study started taking meals in wheel chairs. In addition, 3 subjects with dementia, weak legs and a history of institutionalization were able to walk, go shopping, knit (they even taught knitting), and perform other tasks. Moreover, increases in HDS-R points were observed. 3.No subjects were treated with medicine. All subjects were provided with mattresses (providing a gap from the floor), and 2 of them were provided with moutons (2 sets were used for each subject, and were changed every 2 - 3 days). 4.In this study of a simple double wake-up method, in which subjects experienced the same conditions every day, subjects did not experience drops in blood pressure when getting up, and they became accustomed to getting up independently.
