Five healthy older persons and five hemiplegic patients in Hokkaido were examined the daily amount of walking and the daily physical activity, and assessed the lower extremity function at winter and summer. Three-dimensional accelerometer were used to measure daily physical activities. Lower extremity functions ; balance, gait, strength, and endurance were evaluated by examining ability to stand with the feet together in the side-by-side, semi-tandem, and tandem positions, time to walk 10m, and time to rise from a chair and return to the seated position 5 times, and distance to walk in 6 minutes. At winter, hemiplegic patients reduced the daily amount of walking due to the frozen surface, and time to walk 10m were prolonged. An additional reduction of the daily physical activity in hemiplegic patients led to the remarkable deterioration of the lower extremity function. In healthy older persons, there were no change in the daily amount of walking, the daily physical activity, and the lower extremity function between winter and summer. Hemiplegic patients in Hokkaido need some considerations to maintain the lower extremity function at winter.
