Research Abstract

やせや肥満の背景には、食事や運動だけでなく、喫煙や飲酒・睡眠などの生活習慣が関連している。多くの研究では、対象者の飲酒量と喫煙量には相関関係が見られるが、適量飲酒者の喫煙が糖尿病発症に及ぼす影響は明らかでない。そこで本研究では茨城県民の住民健診の縦断的データを使用し、飲酒・喫煙を層別化して組み合わせた際の糖尿病発症リスクを検討することを目的とした。結果は日本疫学会、日本糖尿病学会において発表された。【方法】対象者は、1993年に茨城県検診を受診した40-79歳の男性63865名とし、糖尿病罹患者(空腹時血糖値≧7.0mo1/1または随時血糖値≧11.1mol/lまたは糖尿病治療中)(n=3116)、データ欠損者(n=346)、継続受診しなかった者(n=16809)、喫煙または飲酒をやめた者(n=20671)を除外した22923名を2007年まで追跡した。喫煙は「なし、タバコ20本/日未満、20本/日以上」、飲酒は「なし、アルコール摂取量15g/日未満、15g-30g日、30g/日以上」に分類し、両者の組み合わせによる糖尿病発症リスクを検討した。リスク比はCox比例ハザードモデルを使って計算した。解析にはSAS9.3を使用した。【結果】非喫煙かつ非飲酒群と比較して、一日にタバコ20本以上の喫煙かつアルコール摂取量30g以上の群では糖尿病発症のハザード比は1.42(95%信頼区間 : 1.21-1.67)となった。また、非喫煙群では、J字型の傾向を示した。一方喫煙群では、タバコ20本/日未満の群では、それぞれ1.26(0.94-1.69), 1.27(1.04-1.54), 1.38(113-1.68)、20本/日以上の群では飲酒量が増えるにつれて直線的に糖尿病発症のハザード比が上昇した。【総括】喫煙と飲酒が重なることで、どちらかのみを摂取している場合よりもさらに糖尿病発症リスクが上昇した。また、適量飲酒による糖尿病発症リスク低下の効果は、喫煙によって打ち消される可能性が示唆される。
