Research Abstract

本研究は、脳卒中片麻痺患者のリハビリテーションの発展を目指し、片麻痺からの運動機能回復を効率的に促すための生理学的機序を明らかにすることを目的として遂行している。
今年度は、本研究において得られたこれまでの研究結果を学術雑誌に掲載させることが出来た。この結果では、本研究の重要な要素としてあげた、左右の大脳半球間の半球間抑制について、運動遂行に伴って運動速度や運動の局面に強く影響を受けることが確認された。また、もう1つの研究課題である経頭蓋直流電気刺激(tDCS)が運動機能に及ぼす影響についても研究成果が得られた。両側の大脳皮質にそれぞれ陰極刺激と陽極刺激のtDCSを与えると大脳皮質運動野の興奮性変化に伴って両手協調運動における左右の運動貢献度が変化することが明らかとなった。これは脳卒中によって起こる片麻痺患者において、リハビリテーションの効率を変化せしめる重要な知見になり得ると考えられる。しかしながら、これらの一連の研究成果を脳卒中患者を対象とした実験において完全に検証しきれなかったことは反省すべき点と認識している。これに対して、派生的ではあるが、一部の研究成果は特別研究員として滞在した米国ピッツバーグ大学の研究において脊髄損傷患者を対象とした実験に応用することが出来た。この研究では脊髄損傷患者において手指の巧緻性トレーニングと皮質脊髄路の神経可塑性を惹起する末梢刺激の組み合わせにより左右大脳運動野間の神経活動バランスの是正を念頭にした運動機能の回復が見込まれることが示唆された。
