Research Abstract

ナトリウム利尿ペプチド系はレニン・アンジオテンシン・アルドステロン系に機能的に拮抗する心保護作用を示す系であるが、そのANPとBNPの受容体であるGC-A欠損マウスに急性心筋梗塞を作成しGC-A系の急性心筋梗塞の予後、慢性期リモデリングに与える影響を検討した。
GC-A-/-マウスに、急性心筋梗塞を作製して、その予後を発症後4週間、野生型マウスと検討すると、有意にGC-A-/-マウスで予後が悪かった(死亡率:11/34 vs 21/30,p<0.001)。剖検にてその死因を検討するとGC-A-/-マウスでは明らかに心不全死の割合が有意に高かった。また、4週後のリモデリングでは、GC-A-/-マウスの方が有意に心重量が大きく、また、心エコーにて得られた左室後壁厚も有意に大きく、組織的にも非梗塞部位での間質の線維化が顕著であった。
GC-A-/-マウスは食塩非感受性の高血圧を呈するモデルであるので、ヒドララジンを投与し、野生型マウスと同程度まで降圧した条件で同様に急性心筋梗塞を作製して検討した。ヒドララジン投与群でも、急性期の予後も慢性期の梗塞後リモデリングもGC-A-/-マウスでヒドララジン非投与群と全く変わらず、高血圧そのものが予後や梗塞後リモデリングに影響しているという仮定は否定された。以前の研究で、GC-A-/-マウスで認められる、高血圧と心肥大および間質の線維化は、GC-A-/-マウスとAT1-/-マウスを掛け合わせたdouble kockout(GC-A-/-・AT1-/-)マウスでは、完全に消失していたので、GC-A-/-・AT1-/-マウスに急性心筋梗塞を作製し検討した。しかし、GC-A-/-・AT1-/-マウスでも、生存率は改善しなかった。しかし、慢性期の間質の線維化はほぼ完全に抑制できた。以上のことから、GC-A系は急性心筋梗塞において急性期に心不全の合併を防ぐ方向に、また、慢性期における梗塞後リモデリングに対しても保護的に働いていることが示唆された。
