Research Abstract

本研究の主な結果は以下の通りである。
1)アジア・南米などの環太平洋地域の調査では、胃がんに占めるEBウイルス関連胃がんの割合が南米地域で高く、アジア地域で低かったことから、生活習慣、民族的背景との関連が示唆された。一方、ペルーは唯一の例外で、南米の周辺諸国と比べるとEBウイルス関連胃がんの割合が非常に低かった。
2)EBウイルス関連胃がんは、p16蛋白発現抑制が顕著であり、宿主DNAのhypermethylationが示唆された。またEBER陰性胃がんと比較して、膜β-カテニンの発現が比較的保たれ、核β-カテニン陽性例が多い傾向を認めた。またEBウイルス関連胃がんでは、MUC2,Cdx2,MUC5ACの発現が少なく、より脱分化した胃がんとなる可能性が示唆された。
3)胃がん由来のEBVと健常者うがい液由来のEBVのサブタイピングを検討すると、チリやコロンビアの胃がん患者ではBamHI-I領域D型とXhoI-kept型優位であった。一方ペルーでは、明らかなD型、XhoI-kept型への偏りは認められなかった。日本と韓国の胃がん由来EBVのgenotypeを調べた結果、A型、F型が主で、これについては文献的に知られる一般人のEBV genotype分布と著差を認めなかった。また、XhoI切断部位が保持されている症例が約2-4割認められ、BamHI-I領域ではごく一部の症例でD型を確認した。
4)鹿児島とコロンビアにおける面接調査の結果や各国の病理学的知見などから、胃底腺領域粘膜の機械的障害がEBウイルス関連胃がんの発症と関係している可能性が示唆された。
5)HPV関連がんの予備的検討では、HPVが検出される肺がん・食道がんの頻度が地域で異なる可能性を示唆する結果を得た。
