Research Abstract

ブラジルを中心とする在外で生活する日系人を対象として、がん発生における遺伝的素因と環境要因との関わりについて疫学的手法を用いて検討している。サンパウロ州における1999年から2001年の3年間の死因統計に基づき年齢調整死亡率(SMR)求め、日本人の統計と比較した。日系人は、悪性腫瘍、自殺死亡が少なく、糖尿病、虚血性心疾患、脳血管疾患、他殺死亡が多いという特徴が観察された。また国勢調査から分母人口の入手可能な日系人1世について1980年、1991年、2000年の3点における死亡率の推移を検討するために、1980年と1991年のデータの整理を行った。乳がんの症例対照研究は、環境要因による発がんの影響を修飾する遺伝素因の役割についての検討を効率的に行えるように、日系人とブラジル人の各々を対象とした2つの症例対照セットに、別の研究費により収集している日本人の症例対照セットを加えた3セットについて、環境要因と遺伝素因についての情報を収集し、両者の相互作用について検討するプロトコールを作成した。関連施設の倫理審査承認後に開始し、これまでに日系人症例109例、ブラジル人症例421例、日本人症例428例、および、ほぼ同数の対照例の調査を終了した。対象者の収集が完了したブラジル人についてBuffy coatからDNAの抽出を開始した。また食物摂取頻度調査票から栄養素摂取量の計算を行った。また平成15年度から開始した食物摂取頻度調査の妥当性研究は、今年度に予定されていたすべての調査が終了し、食事記録のデータベース化・データクリーニング作業を行っている。さらに、大腸腺腫の症例対照研究の実施準備として、米国ハワイ大学および国立がんセンターがん予防・検診研究センターで行われている大腸腺腫の疫学研究と比較可能なプロトコールを作成し、サンパウロ州の病院において研究実施の可能性について検討した。
