Research Abstract

To clarify the pathogenesis of Itai-itai disease, renal tubular function and bone metabolism were examined in 22 subjects with renal tubular dysfunction for 16 years between 1985 and 2001. The patients all live in the cadmium-polluted Jinzu River basin in Toyama, Japan. The mean creatinine clearance had significantly decreased from 62 ml/min in 1985 to 53 ml/nun in 2001. Urinary excretions of total protein, β_2 microglobulin and glucose had significantly increased after 16 years. Raised levels of serum calcium and inorganic phosphate and decreased excretion of urinary calcium noted in the 2001 examination might be due to the decrease in the glomerular filtration rate. Bone mass evaluated by the digital image processing method had significantly decreased from 2.01 mmAl in 1985 to 1.69 mmAl in 2001. Pseudofracture, which is the characteristic sign of osteomalacia, was noted in four subjects on X-ray examination. In three of these four subjects, the glomerular filtration rate and the serum concentration of inorganic phosphate were more markedly decreased after 16 years compared to the other 18 subjects. Consequently, we speculated that decrease in the glomerular filtration rate and serum concentration of inorganic phosphate might play principal roles in the development of osteomalacia in cadmium-induced nephropathy.
