Research Abstract

In order to clarify the function and the efficacy of visiting care service, the research reported here focuses on the function of visiting care service for schizophrenic patients, especially when the symptoms exacerbated, and analyzes the details of care given and the changes in the length of hospital stays and the medical costs two years before and after the introduction of the service, We then divided these into two groups, before and after the introduction of the service, and tested the difference between population means of these two groups.
After visiting care service started, hospital stays largely declined to 102 days, compared to 366 days before it started. However, no change was seen in the number of admissions, which were both 1.2.
Assuming the frequency of outpatient treatment as once every week in the two-year period before the visiting care service started, excluding inpatient period, the total medical expenditure was estimated \4,958,830. And assuming the frequency of outpatient treatment as once every week and that of visiting care service as once every week in the two year period after the visiting care service started, the total medical expenditure was estimated \2.478,992.
We nave interviewed to 35 patients about specialties and needs of visiting care and analyzed it with grounded theory approach. So we have extracted three core categories [(1) assistances with daily life], [(2) communication for emotional contentment], and [(3) keeping up with medical needs ] from visiting care. Of those categories, category (1) proved that patients regard a cooperation or not as most important, and category (2) proved mat it is so important of respecting for patients' voices that patients need nurses to say "May I help you? "and "Are you in trouble? "
