肺癌脳転移と癌性胸膜炎における血管新生の分子生物学的検討と新規治療法開発

Research Abstract

本年度は、申請者が確立した癌性胸水(胸膜炎)モデルを用い、血管新生因子のひとつであるマトリックスメタロプロテアーゼ(MMP)の関与を主に検討した。癌性胸水モデルとしてヒト肺腺癌株PC14PE6をヌードマウスの尾静脈(i. v.)に接種した。PC14PE6は腫瘍細胞接種35から49日後に肺に多数結節を形成し、さらに大量の血性癌性胸水を形成した。In vitroにおいてPC14PE6はMMP-1、MMP-2、MMP-9を発現していた。選択的MMP阻害剤ONO-4817はPC14PE6が発現するMMP-2およびMMP-9活性を抑制したが、MMP-1活性は抑制しなかった。PC14PE6を接種したヌードマウスに、腫瘍細胞接種目より連日ONO-4817治療を行なうと、肺転移および癌性胸水形成が抑制された。ところが、14日後から治療を開始した場合にはほとんど治療劫果が認められなかった。In situ zymographyを用いた肺転移巣におけるMMP-2およびMMP-9活性の検討では、PC14PE6は肺転移巣で強いMMP活性を発現しておりONO-4817はそのMMP活性抑制していた。以上のことより、肺転移形成および癌性胸膜炎への進展にMMPが関与していることが明らかとなった。さらに、MMP阻害剤は非常に早期より治療を開始した場合には肺転移および癌性胸水を抑制するが、腫瘍が増大した状態ではその進展を阻止できない可能性が示唆された。以上より、MMP阻害剤を用いた治療を確立する上で、腫瘍の進展の程度や治療開始時期を考慮する必要があると考えられた。
