Research Abstract

本態性高血圧症は遺伝的素因の関与が示唆される代表的な生活習慣病であるが、本疾患が具体的にどの様な遺伝子異常を背景としているかは今なお殆ど不明なままである。また本態性高血圧などによる長期にわたる圧負荷はやがて心筋の収縮力低下および細胞死を導き、うっ血性心不全に至る。我々は本研究計画において本態性高血圧症および心不全の発症・進展機構を明らかにする目的で、疾患モデル動物あるいは患者心筋サンプルを用いた網羅的遺伝子発現解析によりうっ血性心不全の病態解明を目指した。Dahl ratは遺伝性食塩感受性ラットであり、高食塩負荷摂取により全身性高血圧・心肥大を発症する。高食塩負荷の持続により比較的短期間のうちに重症心不全へと移行するため、ヒトの心不全発症機構の良いモデル動物といえる。そこで各病期のDahl rat心筋を用いてDNAマイクロアレイ解析(約8800種類の遺伝子)を行い、病期特異的に発現変化する遺伝子の同定を試みた。その結果「心肥大発症に伴って発現亢進する遺伝子群」、「健常期および心肥大期では発現が高いが心不全への移行に伴って発現が低下する遺伝子群」など様々な疾患関連遺伝子を同定することに成功した。興味深いことに後者の中には細胞のアポトーシスを制御する転写因子が含まれていた。本遺伝子産物のdominant negative変異体を発現する組換えアデノウィルスを作成し培養ラット心筋に導入したところ、アンギオテンシンII添加による細胞内カルシウム濃度の上昇がブロックされることが示された。
一方、自治医科大学の卒業生ネットワークを介したDNAサンプル収集を行い、本態性高血圧症に寄与する遺伝子多型の解明を目指した
