Research Abstract

Impact of psychotropic drugs on the foetus and infants
Perinatal period there is a risk of suffering from mental disorders. Clinical concern should be paid when women are prescribed psychotropic drugs in terms of the side effects which might occur to their foetus and infants. This study is aimed to show guidelines for prescribing prychotropic drugs during pregnancy.
Methods:
In our university hospital, we have set up a mental health clinic for pregnant and postnatal women in collaboration with obstetric and psychiatry staff. (1) The authors studied the following drugs and pharmacodyamics of these drugs are shown as (1) daily dose (2) maternal drug concentration during late pregnancy (3) drug concentration in umbilical cord (4) maternal drug concentration postnatally.
【clomipramine】 (1) 75, (2) 58, (3) below 20ng/ml, (4) 54
【sulpiride】 (1) 100, (3) 200, (4) 180
【Haloperidol】 casel (1) 3, (2) 4.1 (3) 3.2 (4) 4.2。 Case2 (1) 6, (2) 8, (3) 6.4, (4) 6.8。 Case3 (1)1, (2)2, (3) and (4) below 1.3ng/m … More l
【Chlorpromazine】 case1(1)25, (2)(3)(4)below5ng/ml. Case2 (1)175mg, (2)464, (3)147, (4)542。
【Paroxetine】 (1)10, (2)(3)(4)below 5ng/ml.
【alprazolam】 (1) 0.6mg, (2)(3)(4)below 40ng/ml
【Lithium carbonate】(1)400mg, (2) 0.17Meq/1, (3) 0.30Meq/ml, (4) 0.30Meq/ml.
<impact on foetus and indfants>
Both antidepressants and neuroleptic drugs prescrobed to the mothers were transferred into foetus through umbilical cord, the drug concentrations were almost parallel between maternal plasma in pregnancy and postnatal period, as well as in umbilical cord. Obstetric outcome and infant development were all normal except for one baby who were sleeping at birth (in infant of the mother; chlorpromazine case number 2 and lithium). They gave birth to healthy babies who showed no acute side effects. The infants are now between a neonate and the first three cases are beyond one years old, showing so far no developmental delay. Studies on prescribing the drugs and outcome of the foetus require a larger number of subjects and longer term follow up period. Less
