放射光血管撮影による末梢血行障害の病態評価と各種治療法の効果判定に関する研究

Research Abstract

四肢末梢循環不全について,細動脈レベル以下の毛細血管相の微小循環を可視化するため放射光血管撮影を用いる研究を計画した。下肢虚血モデルをラットで作成し,これにカテーテルを挿入し放射光血管撮影ができ,血管径100μ前後までの血管床を可視化できることがわかったが,更なる画像の明瞭化のための工夫を要することも判明した。末梢循環の増悪因子として重要視されている喫煙の影響を解明できるように,ラットで喫煙状態にするシステムを確立し,血管攣縮が起こることを確認した。また,手指動脈レイノー現象の臨床での可視化に先立ち,コールドスプレーによる寒冷刺激に対する末梢血管の反応を観察したが,反応性の血管拡張の像を観察できた。肺循環でphosphodiesterase 4 inhibitorが血管拡張を促すことも確認でき,これを用いて放射光血管撮影で末梢循環不全の改善が得られるかどうかを確認する予定でいる。ランゲンドルフ心灌流装置を用いて,ラットの冠動脈の攣縮について観察可能なシステムを構築できた。冠還流量の測定とともにその造影所見を確認しながら各種薬剤の効果を判定できることが分かりつつある。
臨床例についての研究に先立ち,さらなる末梢循環不全に影響する因子に関する基礎実験を要するのと,更に明瞭な画像を得るために放射光血管撮影・画像化システムそのものの改良も重要と考えられた。
