Research Abstract |
The purpose was the evaluation of the effect of the chemo-radiation therapy for esophageal cancer by estimating tumor metabolism of glucose and amino acid using positron emission tomography. 51 cases with advanced esophageal cancer were examined. After synthesizing the tracer'18F-FDG (11C-methionine), 148(370) MBq of it was administrated via vein to the patient. The PET scan was performed 60(30) minutes after the injection when the Uptake of FDG (Methionine) in the tumor was stabilized. The uptake of the tumor was displayed by PET and it was evaluated quantitatively by SUV (standardized uptake value). In the result, the mean tumor uptake of 18F-FDG or 11C-methionine was 6 times higher than that of normal esophageal tissue and the distribution was 2 time to 10 times higher SUV. The uptake level didn't related with the histological type, tumor markers, the effect of 5-FU or CDDP evaluated by hysto-culture drug response assay (H DRA) respectively. The uptake of both tracer were decreased early after the beginning of chemo-radiation therapy and it went ahead of the reduction of the tumor diameter so that the early evaluation of the treatment was enabled. Especially, FDG decreasing in a week from starting of the therapy was significantly related with the tumor reduction 6 weeks after the starting of the treatment. On the other hand, the methionine uptake was related with the effect of CDDP/5-FU therapy and it suggested the usefulness of the tracer of predicting the effect of biochemical modulation between CDDP and 5-FU.
