穿通しない根管の形態と、その対応法について

Research Abstract

根管治療において根尖孔の穿通を妨げる原因は,主に根管の狭窄または根管の湾曲である.本研究では電気的根管長測定器を用いた根尖部形態の診断の可能性を検討した.
ヒト健全抜去歯771歯の歯冠を削除後,根管内に6%NaClO溶液を満たした状態で,#1 ProFile Series 29(DENTSPLY TULSA, USA)を用いて穿通を確認した.根尖孔にファイル先端が確認できなかったものを非穿通根管とした.これらの根管の根管内に6%NaClO溶液を満たし,ファイル停止位置での根管のインピーダンス(測定周波数0.4kHz, 8kHz)およびRoot ZX(モリタ,大阪)のメーター値を測定した.さらに同じ位置でファイルを固定して撮影したContact-Micro-Radiographyに基づいて,ファイル停止位置までの距離とインピーダンスおよびメーター値について比較検討した.
非穿通根管数は,1416根管中70根管であった.このうちファイル先端が根尖から3mm以内で,根尖部が狭窄している群は23根管,狭窄していない群は28根管であった.インピーダンスの比は二つの群で有意差が認められた(P<0.05).またRoot ZXのメーター値が3よりも振れなかった根管は,狭窄群では69.6%(16/23根管)非狭窄群では25%(7/28根管)で有意差が認められた(P<0.05).ファイル先端が根尖から3mm以内に位置しており,Root ZXのメーター値が3まで振れない場合は,穿通を妨げている原因が根管の狭窄である可能性が高いことが示唆された.
