Research Abstract

神経細胞の興奮にともない、その周辺領域の微少循環系における局所血流の増大、神経細胞の酸素消費に伴う酸化型/還元型ヘモグロビン比率の変化、神経細胞の膨化等の2次的な内因性変化が起こる。内因性光学シグナル検出とは、神経細胞の興奮にともなうこれら2次的な内因性変化が光散乱や吸光などの光学的変化を生ずるという性質を利用し、神経細胞の活動を反射光強度の変化としてとらえるという方法である。体性感覚野や視覚野など大脳皮質からの内因性光学シグナルの検出は過去にも多数の報告があるものの、これを脊髄に応用した報告はまだない。今回我々は、ヒト脊髄への応用のための基礎データを得るため、In vivoラットを用いて末梢神経刺激により脊髄に誘発される内因性光学シグナルの特性について解析を行った。その結果は、以下のように要約される。
1.頚髄において正中神経と尺骨神経の同時刺激により誘発される光学的変化は、C5-C8レベルに認められ、C6-7に最も強い変化を認めた。この変化は刺激開始後約2秒後に正のピークをとった(Fig.2)。
2.続いて、脊髄からの光学的変化の最適刺激強度を確かめるため、刺激の強度を変化させ、得られた光学的変化を観察した(Fig.3)。刺激電流の強さに応じて光学的変化も強くなる傾向が見られたが(刺激強度依存性)、刺激強度が2.0mAを越えると、光学的変化の増強はあまり認められなかった。
3.シグナルの得られる深度を解析するため、光学的切片法、すなわちイメージングする焦点深度を変えていき得られた光学的変化を観察した。このとき光学的変化は頚髄表面から深さ100〜800μmで記録され、深さ300〜400μm前後で最も強い光学的変化を記録した(Fig.4)。
4.得られた光学的変化には照射光の波長ごとに違いが認められた(照射光波長依存性)が、光学的変化の時間経過は605nm、630nmではbiphasic patternとなり、730nm、750nm、850nmではmonophasic patternとなった。さらに605nmの光を利用したときに最も強い光学的シグナルを得た(Fig.5)。
